1> U.S. registration or U.S. insurance documents showing VIN, name, address and expiration date. Supporting documentation must be available at time of delivery, and must have been valid within 6 months of new vehicle contract date.

2> For current VCFS or US Bank V360 leases, a copy of the lease contract can be used. For returning VCFS or US Bank V360 leases, a completed Volvo Odometer Disclosure Statement may be used. The Volvo Odometer Disclosure Statement must be signed by the lessee. 

Retailer generated DMV printout, vehicle title, prior sales or international registration/insurance are not acceptable as supporting documentation as they may provide insufficient information to prove eligibility. Required additional supporting documentation: 

? Copy of Marriage Certificate when new vehicle is registered to spouse of prior owner. 
? Copy of Business Articles of Incorporation when prior vehicle or new vehicle is registered to the individual’s business entity (requires the individual's majority ownership in the business entity) or copy of individual’s IRS Schedule K-1 (Form 11205) (requires individual’s majority ownership as indicated on Line F) for most recent tax year. 
? Proof of address when loyalty is eligible through a member of the same household. Retailer is required to scan copies of supporting documentation into one (1) PDF file** (see below) and upload when applying for promotion on VRC2.

Rebate benefit The Loyalty Bonus is a customer rebate and retailers are required to display the bonus as a benefit to the customer on the separate rebate line on the sales or lease agreement. If a retailer is using a contract that does not have a separate rebate line the retailer has the option to subtract the rebate from the selling price of the vehicle and instead have the customer sign the customer benefit claim form (page 3). This form must be maintained in the deal jacket for a minimum of 2 years subject to a sales audit.
